Hi,
My name is Corey Gilmore. I'm 33 years old, and I'm incarcerated for an armed robbery committed when I was 18. I'm currently taking a course in psychology and to complete my research paper, I'm doing research on the leading causes of stress & depression. I would like to report the findings of 100 to 200 participants. I've chosen stress and depression as a research topic because not knowing how to deal with my stressors contributed to me committing the crimes I committed, and I would like to share my learned coping skills with others, in particular adolescents.

1. What is your age, race, and gender?
2. Does depression, or related conditions like anxiety or substance abuse, run in your family?
3. Have you ever experienced a depressed, sad, or irritable mood for more than a two week period?
4. Have you ever experienced a diminished interest of pleasure in all, or almost all, of your activities?
5. Have you ever experienced extreme feelings of guilt, worthlessness, or low self-esteem for more than a two week period?
6. Have you recently experienced a lot of stress?
7. List the specific sources of stress in your life and rate them on a scale from 1 (not at all stressful) to 10 (stressful).
8. List your daily hassles and inconveniences and rate them on a scale from 1 (not at all stressful) to 10 (stressful).
9. Describe your typical way of dealing with those stressors.
10. List the most important people in your life, whether the relationship is positive or negative. For each person, describe what you get out of the relationship and rate your overall satisfaction with the quality of the relationship from 1 (very satisfied) to 10 (very dissatisfied).
11. Do you have a hobby, career, or other activities that you enjoy or are very skilled at? Describe the activities.
12. In addition to the stressors which you listed, are there any other circumstances in your life that you believe influences your depressive, sad, or irritable symptoms?
