Ok Concerning Visiting

Due to COVID, the state is allowing only Video Visits, an approval visiting form signed by me is necessary. So in order to get approval a potential visitor needs to snail mail me with an address so I can send them the visiting questionnaire (form). Then the signed filled out form is returned to the institution for vetting and either a yes or no by administrations visiting Siurgent staff.

Please for your own protection use snail mail for addresses and phone numbers sorry, as positive may get you unwanted attention.
