REASON FOR CONTACT
Mr. Lindell was provided with IQ testing on 04/08/2011 (Note: This is when he did it. Didn't see him for several months afterwards) to identify his level of intelligence. He was administered four subtests from the WAIS-IV.
RELEVANT HISTORY/PATIENT'S REPORT
Please review PSU file for information regarding Mr. Lindell's mental health history.
Mr. Lindell was administered four subtests from the WAIS-IV IQ test in the Health Services Unit. There were no complications involved with the testing administration. Individual subtest scaled scores were as follows:
Similarities: 16 (Very high)
Vocabulary: 19 (Very high)
Information: 18 (Very high)
Comprehension: 18 (Very high)
Note: These are the age-adjusted scaled scores. See post #93 + #98 for clarification on what they mean. Because only four subtests were done, there may not be enough scaled scores to reasonably determine any of my cluster or full-scale I.Q. scores for the WAIS-IV. Too bad he didn't complete it.
Mr. Lindell's subtest scores were well above average. He demonstrated very high ability for fund of knowledge, conceptual knowledge, judgment, and processing of verbally presented information.
MENTAL STATUS
Mental status of Mr. Lindell was not obtained for the purpose of completing this particular note.
