4. The reason you feel that way.
5. Belief that I will be useful and law abiding if given another opportunity.
6. Describe my improvements in attitude and behavior.
7. State if you can spend time with me doing something positive.
8. Can offer advice or encouragement.
9. Wiling to help provide housing, clothing, transportation, or some other type of help.
10. Must include your name, address, and telephone number
